The Health Service Executive, as the Contracting Authority, is seeking to appoint a suitably qualified Economic Operator to provide an Integrated Community Case Management System (ICCMS) to include implementation, configuration, integration, training and ongoing support and maintenance. The vision for this programme is to support the delivery of care using standardised care pathways and integrated, person-centred care through the implementation of a communication and information case management system. The ICCMS will provide Community Services staff with real-time, electronic access to comprehensive, up-to date and relevant patient health information, supporting informed clinical and operational decision making across the patient journey within Community Services and linking to GP and Acute Services.
