Registration of individuals will require the following. • Collate hard copies of forms. • Ensure SICAP staff have suitable access to maintain support for each registered individual including: a) Individual registration form b) Individual Personal Action Plan (PAP) c) Track progression of all individuals 6 months after intervention d) Complete documentation for input into Integrated Reporting and Information System (IRIS), (i.e., SICAP’s computer-based performance and monitoring system), data as specified in a to c above, for each individual (at least 60 individuals in total.
